Cml blast phase

Hi there i was diagnosed in june 2018 and started imatinib. But it did not respond well and cml reached to blast phase. I started dasatinib along with chemo therapy and came back to chronic. My blast was 40% and now i have 1%. My latest bone marrow shows no atypical cell and no marrow fibrosis. My doc suggest me to have transplant. Is it good to have transplant? I have done my first bcr abl test since diagnosed and it shows 25%. Is it good or bad? Suggestion on this matter please.

You were diagnosed in June 2018 and imatinib failed you. You started dasatinib and was successful in returning to chronic phase (blasts ~ 1%) which is good. Your latest bone marrow shows no sign of CML?, but your PCR shows 25%? That makes no sense. Was your bone marrow tested by FISH?

You may not need to change drugs and certainly not a transplant depending on what is data is actually correct. 1% blasts is in the normal range for patients and non-patients alike.

It depends on how long you were taking Gleevec.  If you only recently (3 months or less) started taking Dasatinib then 25% is probably not the worst result.  However,  If you have been taking it for a long time and are at PCR 25% then it is not an ideal result.  It is good to have returned to Chronic Phase but I would get a lot more information from my doc than you currently have shared before deciding to go for a transplant.  A second opinion is probably something I would do under any circumstance before going that route.  

FYI I was at 23% after 3 months on Dasatinib.  1.8% after 6 months, and 0.111% at 16 months.  So as you can see I missed the 3 month mark, hit the 6 month mark and am trending the right way to get under 0.1% by 18 months.  Some people respond different/faster than others but the trend and rate of decline is something that you should probably look at in deciding what treatment option to go to next.
