Sprycel Reduction to 20mg After 15 Months

Scuba and Friends - My husband is 40 years old and was diagnosed with CML in Jan 2018.  He started treatment on 100mg of Sprycel (Jan 2018), had a ~1 month self-imposed break  in Nov (no medication) and resumed medication at a reduced dosage of 50mg in Dec 2018 (although his oncologist was reluctant to do so) - he has been on the 50mg reduced dosage of Sprycel since Dec. PCR results are summarized below - current PCR is 0.01%. Our next appointment with oncologist is at the end  of March and we would like to suggest a further reduction to 20mg. We’d appreciate your feedback on proposed dosage reduction and link(s) to relevant case studies that we can use during our discussion with our oncologist (since he was reluctant to deviate from the 100mg back in Dec). Thanks!

Maybe 40mg/day would be a compromise your doctor would go for.  Personally, I would want to have a few more PCR results to verify the trend.

A 40mg/day prescription would also give some flexibility in dosing.  That is, if your husband decides a more gradual reduction would be wise.

He has had an OUTSTANDING response. Your intuition of dropping to 20 mg is smart.

The lower the dose, that works, the better when it comes to sprycel. His CML is getting clobbered. No need to clobber his immune system with higher than needed sprycel dose.

I have been diagnosed with CML in Dec 2017 and have been on complete molecular remission since March 2018.  My doctors insist on continuing on 100 mg spycel daily.  i have been reading on this website on the possibility of reduction.

Recently because of some infection had to take antibiotics and was advised to stop sprycel.  Post 3 days of stopping i am feeling really good as if a the body has suddenly become more fresh/lighter/alert ect

when i restart i am tempted to reduce dossage to 50 mg. ofcourse this will be a self impossed decison

any views/suggestions

 

 

I've seen advice that recommends reducing dose six weeks before your next PCR test as that provides timely feedback of the effect of the reduction.
