severe side effects of Dasatinib within 3 doses

Categories:

hi i am shyam patient of cml since 7 years. Past 6 weeks ago as I had tremendous side effects of imatinib 300mg  , my doctor told me to stop it . Recently i have gone through bcr abl test  where it was 64%,  so my doctor told me to  start taking dasatinib 140 mg(70mg morning and night) . I just took 3 doses of it and started to suffer from severe leg and hands pain . My body has become weak.I suffered from extreme dizziness and fatigue and severe head ache like migraine. So again i consulted my doctor , he told me to stop dasatinib and asked me to go through tests like calcium levels, lfr , cbc ,cardio 2d echo, chest x-ray ,vitamin d ,electrolyte levels, and creatinine . I just gave my blood sample and tensed to see the result. I don't understand whether he will prescribe me low dosage of dasatinib or change the medicine and also worried whether they will work on me or show side effects like before ones . If yes what could be the other option?

I saw your other thread and post. It was clear that you did have good response on imatinib and only 300 mg as well. But you report developing severe leg pain. All TKI's - especially imatinib, can cause significant magnesium loss. Magnesium is absolutely vital for muscle function and often shows up as severe leg pain when magnesium levels are falling. I had severe leg pain after only a few months while on imatinib. My oncologist casually suggested I take magnesium. It worked brilliantly. Once I started taking 400 mg of magnesium (in the form of magnesium citrate or taurate 200 morning - 200 just before sleep), my leg pains disappeared - in one night. I have not had any leg pain since. I also switched from imatinib to sprycel (20 mg) and still take magnesium

You almost certainly do not need 140 mg sprycel - that is extraordinarily high. Because you did respond to imatinib (very good), you likely only need a small dose of sprycel as Red Cross Kirk pointed out (20 mg) to keep your cml under control.

Talk to your doctor about resuming Sprycel low dose (20-40 mg)l and track your CML closely. Also add magnesium supplements.

(the good news about magnesium is it will either work or not immediately. You'll know in one night if you had a magnesium shortage. Keep in mind the blood will not show a magnesium shortage - the muscles do)
