Spleen still enlarged after 4months

Hi there,

I had an USG today. My spleen is still enlarged 13.6 cms. I am almost completing 4 months on tasigna.

My PCR - 0.09℅ IS at 3 months.
CBC are in the range- today's value - 8560

Should I be worried? Kindly suggest.

Hi,

0.09% IS at 3 months is a fab result. Anything under 10% is optimal, but your result is slightly more than 100 times better than that. 

How big was your spleen at diagnosis? Has your doctor considered any reason other than CML for contributing to your large spleen?

My spleen @ dx was 22 cm and on Feb 22(usg) it was 14.4 cm as of today it's 13.6cm.

Ideally, I was hoping it to be @normal size. I have been checking WBC count every 2 weeks they are well within normal range.

I have my doc appointment on may 10th.

My spleen was 14cm palpable at diagnosis, and looking back at my notes it took a good month before it retreated back into the rib area. I don't have any further notes as to how long it actually took to return to normal though. I do recall once it had reduced to behind the costal margin (bottom of rib cage) I was much more comfortable.

I know we are not all the same size and shape, but if you started off with a 22cm spleen perhaps it is just going to take a bit longer for it to retreat.

Thanks David. I started feeling discomfort again below left lower rib cage. Worried if spleen started to increase.

Is keeping a tab on wbc good enough to monitor spleen?

Spleen size is best seen by ultrasound, but a good clinician with a well placed finger can tell a lot just by prodding. 

If your bloods are otherwise good, your spleen should reduce in size over time. 

Don't forget not everything is CML related, so it's worth getting checked out to rule out other problems. I think you said you had an appointment on the 10th of May. Make sure to push the issue with your doctor.
